The ration card holder should approach the ration office of his area / at the district and tehsil level, the district supply officer and tehsildar respectively are the authority for processing applications for the addition or deletion of name in the ration card.

The applicant will have to obtain the form from the concerned office (please pay for the form if required) or use a written/electronically typed application on an A4 plain sheet as advised or download the applicable form Use the following link for Link: Form
Kindly submit the complete application along with the required documents to the concerned authority.
After checking the submitted application along with the attached documents the officials will make entries and provide you with the record number for reference.

This application will be sent to the concerned Ward/Gram Panchayat Officer for verification. The concerned officer visits the applicant’s home for verification and approves the request if the applicant is found eligible.
The applicant is informed through SMS at various stages, i.e., advance intimation about the date of visit of the officer at his place of residence for verification of his application, and acceptance/rejection of his application.
Ration card will be issued in 2 – 3 weeks after verification at the ground level or as notified by the department

- Death Certificate in case of death of a member in order to delete
- Document required for Adding member on account of marriage
- Marriage Certificate
- Original Ration Card with husband’s name
- Name Deletion Certificate from the parents of the Bride
- Document required for Adding New Born Child in Ration Card (above 5 years)
- Birth Certificate issued by the local municipal body or competitive authority
- Original ration card of the family with parent’s name
- ID Proof of Parents such as PAN Card, Aadhaar Card
